Audit conducted at the branch on blah blah disclosed that you were in jeans, T-shirt and rubber shoes.

Call to mind that we have called your attention for similar infractions when you were not in uniform on blah blah blah...

In these instances, we imparted to you the significance of not wearing the prescribed company uniform and proper office attire. We also advised you to do your share in projecting a positive image to our costumer. We likewise impressed to you the implications of your infractions. Yet you again disobeyed the directives.

For this, we advised you to wear proper office attire such as slacks, knee level or an inch above the knee skirt, blouse and closed toe leather or synthetic shoes with heel during wash days. Should you still wear jeans, T-shirt or rubber shoes in the future, then we will impose a more severe sanction, including SUSPENSION.

Let this therefore serve as final lesson and a last written warning.
